Our doctors and staff are proud to be a team whose primary mission is to deliver comprehensive periodontal treatmentand dental implant services. Your dental health matters to us, and we want to ensure that all aspects of your periodontal care are handled in the most responsible matter. In order to assist you with your investment in your own dental health, we have outlined key aspects of our payment policy below.
Emergency/Single Visit Payments
For single-visit procedures, payment is due in full at the time of service.
For your convenience we accept cash, checks, money orders, debit and credit card payments (American Express, MasterCard, Visa and Discover).
Multiple Visit Payment
Payment for your procedure is due at time of service.
For your convenience, payments may be made with cash, personal check, money order, bank draft, Debit card, American Express, Discover, Visa or MasterCard.
Insurance
Our staff makes it a priority to provide patients with accurate insurance coverage estimates based on information available to us. However, patients should note that some insurance companies offer many different dental plans and can change benefits, co-payments and deductibles throughout the year. Also, some insurance companies will not give out fees until after the treatment is completed. At times, this makes it almost impossible to accurately estimate our patients’ insurance co-payment. As a courtesy, we ask that you keep us informed of any change to your insurance. It is important that all information about you and your insurance remains current.
Co-Payment
Some dental plans reimburse 100% for only some services. The majority of dental plans reimburse approximately 30%-80% of treatment costs. With this in mind, we will ask that any co-payment fee be paid at the time of treatment.
Private & Group Dental Insurance Plans
As a courtesy to our patients with dental insurance benefit plans, we will submit the necessary claims forms, receipts, and other information with your consent to your insurance company. Upon receipt of an insurance payment, any balance due will be billed to you. If you have paid an excess towards the co-payment, it will be refunded to you.
